 The following six-component program serves as the model utilized to foster stability for partipants: - Orientation: The first step of orientation takes place when the social work/casemanager or client makes a phone call to Hands of Diversity Service Center. If the client passes this initial screening, they will be invited to come to the Center for intake.
- Needs Assessment and Action Plan: The Program Director will conduct an in-depth interview and assessment with the client. Based upon the intake and the assessment, the Program Director and the client will design a written Action Plan that meets the individual's needs.
- Progress Tracking: Each day, the staff will review progress in carrying out the Action Plan to determine where strong advocacy and/or intervention should occur.
- In-House Support Services: Throughout a client's stay, the center will provide many opportunities for residents to learn and share more about a variety of topics. Topics for adults will include: Basic healthcare services, nutrition and sanitation, Life Skills Training, Budgeting and money management, Psychotherapy, Child Care, Permanent Housing Search, Job/self-employment training.
- Resettlement: Resettlement assistance will be provided for all individuals who manifest their need.
- Follow-up: HOD staff will contact former residents quarterly to learn of their progress and any continuing needs.
